This might sound rude, but is it just me, or does the list make the word SNPFU? - "Situation Normal, Programmers Fouled Up", come to mind... not that anyone is perfect.

The one on includes/requires/etc that is apparently so prevalent with PHP, is actually one I've seen in action while auditing stuff that's already been deployed. To the point in fact, that some people I know have gotten a tad paranoid about what they import, and where they upload edits lol.
