I'm not sure of the origin of T & A is, other then that you'll find such an expression in most countries vocabulary (distracting idea!) but R & R is derived from a military term, where ever it coming from aside (US || UK), it is at least commonly known in the USA as Rest & {Relaxation, Recreation, Recuperation}.

When I worked in television starting in the 70s, it was used to tell what was necessary to boost ratings and interest in a show. Whether it actually originated there, I don't know.
