The syspatch(8) tool stores its patches in /var/syspatch. If this is a "default" install on a disk drive of 8 GB or larger, /var should have its own mount point.
Select the correct options for du(1) and you can quickly find the culprit. See it's man page for an explanation of these options.
# du -hxd 1 /
My best guess: /dev may hold an accidental normal file rather than all special files. It is not uncommon for an inadvertent typo by a superuser using dd(1) to create a new file in /dev, thinking they have written data to a device.
