Default Who needs complicated software when a simple hardware solution exists?

Half of people plug in USB drives they find in the parking lot

Apparently stuxnet got into an Iranian nuclear facility by the same method, so that's a simple hardware solution and complicated software.
Confessions of a Cyber Spy Hunter: Eric Winsborrow at TEDxVancouver (skip to 7 minutes if you don't want to watch the whole (quite interesting) talk, Zero Days is an interesting movie too)

A google translation of https://tweakers.net/nieuws/83028/ha...e-bij-dsm.html

Hackers scattered USB sticks with spyware software at DSM

Criminals have tried to steal log-in data from chemical company DSM. They did this by placing USB sticks with spyware software in a DSM parking lot, in the hope that employees would stop in their computers.

Data criminals have attempted to get passwords and user names from the chemical company DSM. They had placed several USB sticks in the company's parking lot in the hope that employees would stop in their work computer. The software on the memory stick was able to send log-in data to an external website.

However, the attempt was unsuccessful; one employee found one of the USB sticks and brought him directly to the IT department. There the malicious software was detected and the IP addresses with which the software communicated were blocked. After that, the other USB sticks were detected in the parking lot.

A spokesperson for DSM reports to the newspaper De Limburger that the company did not file a report. It would be 'a rather clumsy attempt at data theft'. However, a memo has been issued which once again stresses to employees that they do not insert unknown USB sticks into their computers.
