Default OpenSMTPD and upgrading

If one already has OpenBSD 5.7 installed, does one have to track -STABLE to get the latest release of OpenSMTPD?

No, sorry, you will have to move to -current to obtain newer functionality prior to 5.8 release. By definition, -stable never includes any new features or functionality.* It only contains patches that affect reliability, availability, and security.

The only difference between -stable and -release+errata is that -stable may include additional patches which are not of sufficiently broad impact to justify an errata posting.

* The -stable branch includes the ports tree. There may be new features in a -stable port, only because an upstream project has addressed a critical CVE with a new version of their application.

You could always compile a new release. It seems that tarballs are regularly uploaded to https://www.opensmtpd.org/. They could depend on features only available in -currnet libs, but generally you'll be informed of this because the compilation will fail.
